Scott, Make sure if you are using DC power that you are properly grounded. We ran into a problem very similar to yours, but once the chassis ground was properly established all the problems went away. Since these modems were installed they have been giving about 14% lost carrier rate. Since we discovered the problem we've been changing firmwares and trying to get this down but nothing seems to be working. Currently the tc is loaded with software version 5.3.110. We put the old pm4's in place and the lost carriers drop to 1-2% maximum, but with that another problem arises, we've had users calling in and complaining their connection speeds have been a lot slower. I'm thinking the pm4 negotiates a more stable speed for the modems than the tc1000's do and thats what causes the lost carriers in the tc1000 but i don't know.
