I've had the same problem and have been getting the run around. They told me that I had to pay for another year and update my credit card info. I told them that the DVD was the bonus for the year of membership that just passed (by D&W's own admission, the DVD release was delayed for several months) not the upcoming year of membership and they had better send it to me whether or not I renew. I'm still waiting to hear back on this.
If I need to update my credit card and pay again, that's fine. But the DVD is for the last membership I paid for.
I've been very frustrated with the whole process as "Fullfilment" kept telling me to fill out the form, yet each time I went to the site, I got a page that told me I'd already updated the form. I never had an opportunity to fill out a form of any kind.

people have been getting email confirmations from the Sting online store
as they are being sent. I guess that the shipping is being done in the
order the DVD forms were filled out, but it still seems to be taking a
while to get them shipped. It turned out that my DVD was not sent since
their records showed me as not paying for my second year yet. This was
wrong and my account page on Sting.com clearly showed that. I just paid
for year three yesterday! Anyway, I got another email back today stating
that they had cleared everything up and that my DVD was on the way and
that I should receive it next week.
