Get a cheap caliper from the auto parts store or harbor freight, and get an accurate hole diameter and hole spacing measurement. Then you can go to the different sailboat hardware company websites, download there full catalog, and see what matches. You could also try contacting the people at Duckworks (duckworksbbs.com) and see if they stock something with the measurements.

Does anyone know where I can order a replacement horn cleat for my main sail down haul? I ordered the smallest cleat available from West Marine but the holes do not line up with the existing cleat.
