I use the horn cleats for main and jib halyards. I use the original clam cleats for the topping lift and spinnaker halyard. Mostly because they are there. The topping lift cleat had to be replaced. The plastic wears.

I stand in the hatch opening or maybe sit on the side of the cabin top.
I installed Clam Cleats above the horn cleat, and moved the horn cleats a little lower. I think they are the side entry cleats: CL 217 Mk 1 (Starboard) & CL 218 Mk1 (Port). I can use one hand to feed the bolt rope in the slot and use the other to "ratchet" the halyard as I raise the main. I use the horn cleat as a turning block, then secure the halyard to the horn cleat. I do not trust a clam cleat to tie off the halyards.
